There are over 2,000 transit bus companies in Japan. The largest mission for transit buses is to safely transport their passengers to their destinations. Since the 1990s, the fast growing motorization caused decline in the demands for transit buses and about 70% of the businesses faced losses. The trend turned the management eyes to concentrate on restoring their businesses without sufficient attention on securing safety. In our study, we carried out hearings to five transit bus companies that experienced serious accidents during the years of 2005 to 2015. This paper summarizes each accident, identifies factors in the background and measures for preventing accidents at the time, and evaluates how effective those measures were. The paper then shows how to construct a new system for safety management that incorporates lessons learned from severe accidents. Our development extends its probe of cause analysis all the way to the root causes and works their lessons into the system.
